THE CORNER FAMILY SURGERY WILL BE CLOSING ON WEDNESDAY 8th MAY 2024.
Limited doctors are available to provide medical care until the closure date, however you need to follow the following steps as a matter of urgency to ensure continuous medical care.

Find a new doctor

Sign a release of medical records with your NEW medical practice. The practice will then send this document to us.

Contact us to pay the transfer fee of $44.00
Once these steps are completed, your records will be sent to your new practice.

Please note it may take up to 10 days to arrange transfer of your medical records.
ADVISING PATIENTS
Patients who have visited the practice in the past 12 months, and a valid email address is held, have been sent an email. If no valid email address is held by the practice, a letter will be sent advising of our closure.

Dr Collier is currently not taking on new patients. She will only be working 3 days a week at Invitation to Health and will only see her regular existing patients.
If there is room, she may open her books at a later stage.

Surefoot Podiatry will be moving to Invitation to Health, 256 Henry Parry Drive, Wyoming. The proposed start date is Tuesday 14th May 2024.
Patients with existing appointments will be contacted closer to their appointment date to confirm the time and new location.

The pathology company Australian Clinical Labs (ACL) will continue to offer services until Wednesday 8th May 2024.
