If you require records please complete the following steps:

Find a new doctor

Sign a release of medical records with your NEW medical practice.

Have the new practice email the request to admin@cornerfamilysurgery.com.au

You will be contacted to pay a transfer fee of $44.00
Once these steps are completed, your records will be sent to your new practice.
Please note it may take up to 14 days to arrange transfer of your medical records.
ADVISING PATIENTS
Patients who had visited the practice in the past 12 months, and a valid email address was held, had an email sent to them. If no valid email address was held by the practice, a letter was sent advising of our permanent closure.
THE CORNER FAMILY SURGERY HAS NOW PERMANENTLY CLOSED.

Surefoot Podiatry
Surefoot Podiatry will be moving to Invitation to Health, 256 Henry Parry Drive, Wyoming. The proposed start date is Tuesday 14th May 2024.
Patients with existing appointments will be contacted closer to their appointment date to confirm the time and new location.
